We have now started a new system for all of our long term conditions patients.  You will receive an email usually around the month of your birth, although this year will be different as we try and catch up for the past year (We have recently sent emails for patients with a birth month of December & January). The email contains a link which will ask for your date of birth (ddmmyyyy) as a passcode. Once open the link will contain a letter and a Blood Test form if you require any blood tests. Please follow the instructions as to booking any appropriate appointments.
